Treatment protocol for injection lipolysis with Nano-PPC and GLY

The lecture was developed together with Margrit Lettko, who unfortunately could not be present due to illness. Dr. Grübmeyer had already carried out the first tests with the new injection solution several years ago, so he was the right speaker for this topic because he could report from his own experience.

His presentation was divided into 3 parts:

  1. Comparison of the status of our previous compound with the current status of the new injection solution.
  2. Presentation of further studies on the new solution, namely animal studies with 2 additional animal models (rabbit and guinea pig), half-side comparison of the old compound and the new solution by Dr. Hasengschwandtner with 7 patients, and demonstration of treatment results documented by some members.
  3. Discuss the right strategy of introducing the new injectable.

It became clear that the new injectable will be introduced as a complement to the compound because it gives us several advantages, namely shortening of treatment intervals and reduction of side effects, while still using the compound for tightening of lipolysed areas. A first proposal of a combination protocol is currently being developed and will be brought to your attention as soon as the new Injectable is available.


In Vitro and In Vivo Investigations of a New Phosphatidylcholine Combination with Glyzzheretic Acid (GLY) instead of Deoxycholic Acid for Use in Injection Lipolysis

Prof. Prantl and his team have accompanied the network activities since the creation of our organizational structure with many studies, first on Lipostabil, and later on our new injectable. The interesting part of his presentation was the comparison of the study results on both injectables. Similarities and differences emerged, which will make it easier for us as a network to develop the right strategy for the introduction of the new injectable.

What is clear is that the new injection solution showed very high lipolytic activity and that no inflammatory parameters were detected in the animal test. The apoptosis assay also carried out is not yet so conclusive that this can already be firmly established as a mechanism of action, although clear indications point in this direction.


Statistical evaluation of optimization of results by combination of cryolipolysis and injection lipolysis

The first publications from 2016 and 2017 by Prof. Faulhaber and colleagues have already convinced from the results of this therapy combination. Also, the side effect profile is in no way worse than with the two singular therapies, because the tissue is already anesthetized by cryo and therefore the treatment with lipolysis is facilitated for the patients with few side effects. In addition, Prof. Faulhaber showed a statistical processing of the results of the working group. It was possible to evaluate the results of the combination statistically significantly better than the results of the individual therapies.

In addition, it was even possible to make a statistical prediction about the improvement, which is quite helpful for patient counseling.

Thread Lift

The T-ICONS – the NETWORK protocol: Presentation of the complete standardized treatment protocol for the thread lift.

T-Icons means: Thread Indication Coded Operational Network Standards
With this catchy name we want to name the new network treatment protocol in the future. In cooperation with Dirk Brandl, Dr. Mikowsky has worked on structuring and systematizing his practical experience with the thread lift over the past two years, has attended numerous training courses in Moldova, Tbilisi, Istanbul, Milan and Germany, among other places, and has reviewed the existing literature.

The difficulty was that there is a scale of severity only for one aspect, that of skin aging (Glogau scale), while the even more important aspects of ptosis and volume have not been classified by severity at all so far. These two aspects ultimately condition the contour changes of the face. To facilitate this, 4 types of ptosis-volume combinations were determined, with 2 types in particular being excellent for thread lift.
Dr. Mikowsky then described his suture lift algorithm consisting of auxiliary lines, the Line of Ligaments, traction vectors, entry points, end points to which the sutures should be advanced, and the correct suture position and depth. This systematization allows the user to get a clear idea of the contour deficits and then determine the correct therapy by the sutures – always in combination with other procedures.

PRP & Needling – a new treatment protocol for the treatment of skin aging of the hands

To change the skin status in the aging process, 3 regions are important, face, neck and hands. Dr. Koenen briefly introduced needling as a recognized therapy by showing that since 2018 alone there are 126 publications listed in Pubmed that deal with needling. He then described the results of some interesting publications on new combination therapies with needling such as PDT and needling for alopecia areata, needling plus minoxidil for hair loss, or needling and vitamin C for melasma treatment. In the second part of his presentation, he went into the actual topic of treating the hands. He “pads” the back of the hand with tumescent fluid as it is also used for example in liposuction. He always combines needling of the back of the hand with PRP by first performing needling and then incorporating the PRP. His protocol includes 3 treatments at intervals of 4 weeks.


Statistical evaluation of a questionnaire on the use of PRP in aesthetics

Prof. Stollwerck presented the first evaluation of a questionnaire developed by him, Steffen Giesse and Dirk Brandl. Although only 22 members answered the questionnaire, a very large number of treatments (9,800) were documented. All respondents treated exclusively aesthetic indications. What was particularly surprising was the very positive evaluation of the treatment results with a total of more than 68% very good and good ratings. Main indications were hair, followed by skin, scars and eye circles. A publication on this is in preparation.

The company YCellBio, whose kit is used by the network, has dug deep into its pockets to have a comprehensive analysis of the plasma fractions of the centrifuged blood carried out by the laboratory MMD from Magdeburg under the direction of Prof. Brigitte König. Such an analysis is currently unique worldwide. Of course, we were interested in questions about platelet fractions and the detected growth factors. These questions were first formulated in detail by Prof. Stollwerck, who was also present at the PRP production in Magdeburg. Of course, only first results could be named in the presentation, because the analysis is not yet completely finished. Nevertheless, the results were very interesting and shed a significant light on grandiose advertising statements of other kite manufacturers. It was shown that neither in PRP nor in PPP the platelet content increased significantly, but very strongly in the so-called buffy coat, which is why it should be used in the aesthetics according to the manufacturer’s instructions. The common conclusion of all users was that plasma therapy should consider mechanisms of action other than those known to date.


Optimizing the effect of PRP treatments through prior analysis of the patient’s blood

This question is definitely exciting and justified – which also became apparent in the participant discussion. It seems that every now and then there are no good results when treating with PRP. This can of course have several causes. In any case, one of the causes can also be mitochondrial dysfunction. Prof. König was able to explain very clearly how such a dysfunction can occur. For this purpose, the network has included a dietary supplement in its program that is capable of increasing the general activity of the mitochondria. For mitochondrial medicine, Prof. König’s studies open up even more perspectives, because an exact diagnosis of mitochondrial activity makes it possible to test specific active substances in the laboratory, tailored to the patient, and therefore to develop an individual composition for the therapy. For PRP treatment this seems to be necessary only for those patients who suffer from chronic civilization diseases, otherwise the preparation Cell Energy (ATP) recommended by Prof. König can be obtained through the network.

Photographic and medical conditions

In most practices, there is a risk of incorrect or insufficient image documentation of the services provided. The reasons for this neglect are manifold: incorrect selection of the room, several assistants carrying out the documentation, ignorance of the conditions of good documentation as well as ignorance of the technical requirements.

Ernst, alter ego of photo engineer and network speaker Dirk Brandl, assistant in an aesthetic practice, tells in an amusing way how he acquired the standards and is able to present all important aspects that lead to the fact that the viewer of before and after pictures can concentrate exclusively on the result of treatments. The standards he refers to are the basis of the general rules for taking documentation photos in aesthetic medicine that are valid today.

This video workshop teaches both physicians and assistants all the important
basics in an easily reproducible manner. Special emphasis is placed on the implementation in one’s own practice, which should do without its own space and large technical equipment.

This future-oriented presentation showed the manifold new possibilities of adult stem cells without neglecting the dangers that can accompany them. In this context, it was important to note that the development of stem cells into differentiated cells has not yet been fully controlled. Prof. Kaiser showed publications on complications caused by incorrect differentiation of stem cells. There are now some kits on the market and in some countries clinics, e.g. in the USA over 500, which are already advertising stem cell therapies.

For Prof. Kaiser, however, it is important that research results on the controllability of the differentiation process of stem cells should be given before any application. For the near future, he relies on 3 building blocks that further develop aesthetics, namely combination therapies, in which the network meanwhile plays a leading role, on the repurposing of drugs and gadgets as well as the use of paracrine effects starting with PRP via cocktails with growth factors up to conditioned media and micro RNA.
